Preparing for your visit:
We are looking forward to seeing you in the clinic. To make your visit most productive, here’s some information about how to be prepared:
Insurance:
Before scheduling your appointment, we strongly recommend you contact your insurance company to verify that the Sierra Rheumatology and Rheumatology Center or the doctor you plan to visit is included in your network.
If necessary for insurance purposes, your primary care physician can fax a referral to: 916-781-2029
Your insurance company will also be able to inform you of any co-payments, co–insurances, or deductibles that will be your responsibility. If you proceed in scheduling an appointment and your health insurance benefits do not participate with SRI, your out-of-pocket liability may be higher. We will contact you regarding your coverage and patient liability.
At this time, we are in network with the following insurances:

- Covered California (EXCEPT as mentioned above)
- Medi-cal
- Partnership Health plan
Sorry , we are currently not accepting patients with Medi-Cal. This might change in near future. If you have any insurance related questions, please call our office to confirm PRIOR to the appointment, as there are some other insurances where we are not in network. We shall work with you to accommodate to the best of our ability.
